Responsibilities

  1. Conducts a medically prescribed therapy program for patients involving exercise, massage, heat, water, light and electricity.  Applies diagnostic and prognostic test results to muscle, nerve, joint, and functional.  Directs and aids the patient in active and passive exercises.  Records and reports patient progress.                                                  

     

  2. Provides therapeutic treatments and recommendation of equipment needs as applicable and discusses ideas with supervising physical therapist
  3. Advises and consults with agency personnel as applicable
  4. Prepares clinical notes
  5. Participates in in-service programs, mandatory agency staff meetings, case conferences, educational offerings, and Agency training programs
  6. Plans of discharge with discussion with physical therapy case manager
  7. Contacts Supervisor/Case Manager when additional modalities and/or services are required
  8. Participates in established Agency committees as requested by Agency
  9. Utilizes the Supervisor as the first resource when problems or questions occur in the field, including charting guidelines, coordination of disciplines, and supervision of home health aide services
  10. Maintains Agency property, supplies and equipment in a manner that demonstrates ownership and accountability
  11. Demonstrates infection control per OSHA guidelines and Agency policies
  12. Ensures compliance with the Medicare Conditions of Participation and any other regulatory requirements
  13. Maintains a professional appearance, treats others with care and respect, and demonstrates excellent customer service
  14. Possesses and maintains a current CPR certificate and Tuberculin testing. Remains current for all other Agency mandatory items
  15. Performs other duties as assigned.

 

Qualifications

Education/Experience: Graduate of a Physical Therapy curriculum accredited by the American Physical Therapy Association (APTA) and/or the Council on Medical Education and Hospitals of the AMA.  Current licensure as a Physical Therapist Assistant in the State of employment. Minimum of one year recent appropriate clinical experience required.  Home health care experience preferred.

 

Skills/Abilities: Demonstrates good clinical skills in the treatment of neuromuscular and musculoskeletal problems. Possesses good oral and written skills.  Must be a licensed driver with an insured automobile in good working order.
