Responsibilities

Identifies and analyzes the social/emotional factors underlying client illness and communicates these factors to the health team. Assists patients and their families in understanding and accepting treatment of the temporary effects of illness. Assists with financial, community resources so that facilitation of recovery is acquired.

 

  • Assesses psychosocial status
  • Develops the plan of care in consultation with the physician and other care team members
  • Assists other team members in understanding the social and emotional factors related to the health problems
  • Works with family as needed
  • Prepares clinical and progress notes
  • Utilizes and providing client/family with appropriate community resources as needed
  • Plans for discharge
  • Consults and coordinates services with other disciplines
  • Participates in in-service programs, mandatory Agency staff meetings, case conferences, educational offerings and training programs as required by agency.
  • Maintains a professional appearance, treats others with care and respect, and demonstrates excellent customer service
  • Utilizes the Case Manager as the first resource when problems or questions occur in the field, including charting guidelines, coordination of disciplines, and supervision ofhome health aide services.
  • Participates in quarterly utilization review and chart audits for OBQI/OBQM as requested 13. Ensures compliance with the Medicare Conditions of Participation and CHAP standards and any other regulatory requirements.
  • Maintains Agency property, supplies and equipment in a manner that demonstrates ownership and accountability.
  • Demonstrates infection control per OSHA guidelines and Agency policies.
  • Possesses and maintains a current CPR certificate and Tuberculin testing. Remains current for all other Agency mandatories.

Qualifications

Master’s Degree in Social Work (MSW) from a school of social work accredited by the Council on Social work Education; qualified by the State of employment for licensure as a Certified Social Worker (CSW). One year of social work experience (post master’s) in a health care or related setting.

 

Knowledge of social work standards of practice and ethics. Experience and knowledge of community resources and counseling techniques. Strong interpersonal and communication skills with an ability to work within the team concept. Understanding of the role of social work in home health care and

reimbursement guidelines. Must be a licensed driver with an insured automobile in good working order.
