bannerThe Home Care Specialist will perform an inside sales function by developing relationships with referral sources (past or present). He/She will be expected to generate referrals and contribute to the overall market share of Home Care and Hospice. He/She will be responsible for making outbound calls to generate referral requests and act as a Liaison between patients, families and their physicians.


  • Required to maintain an average of 50 outbound calls per day to reach prospective patients
  • Manage prospective, current and former patients to determine elegibility for home care services 
  • Develop strong and lasting relationships with patients and community resources/caregivers
  • Perform non-clinical assessment over the phone to determine patient eligibility 
  • Coordinate referral request with appropiate department to follow up
  • Lead management from internal database 
  • The Home Care Specialist will be responsible for developing relationships with referral sources, discharged patients, and building hospice/homecare market share
  • Identifies individual patient’s/customers special and diverse needs and modifies interactions to satisfy them
  • Delivers patient/customer service seamlessly across system boundaries
  • Ensures patient/customer/family confidentiality and respect
  • Have the ability to sell programs offered by Guardian Angel Home Care and Hospice



  • High School diploma
  • Minimum 1 year experience in home care related field
  • Computer skills: Microsoft Excel, Word, Outlook, CRM software, HCHB software
  • Strong customer service, organizational, and interpersonal skills with excellent multi-tasking and prioritization
  • Professional demeanor, self-motivation, and dependability
  • Ability to communicate effectively with all personnel levels within the organization
