Overview

Using HomeCare HomeBase, coordinates and documents scheduling and

related Agency activities. Provides office support to professional/administrative staff. Collects and

maintains data necessary for accurate reports.

 

 

 

 

Responsibilities

  1. Completes requested schedule for new patients, pending authorization, orders for ROC visits, follow-up, reauthorization, resumption of care, soc/recert.
  2. Verifies schedule ROC oasis collection.
  3. For transfer to inpatient facility, verifies schedule for continuation of services, faxes to doctor.
  4. Processes missed visits and notifies physician. Reschedules visits.
  5. Processes visit time changes.
  6. Processes reassigned and declined visits.
  7. For discharges, verifies schedule data collection and continuation of services.
  8. Runs agency summary report and follow-up with staff.
  9. Reviews scheduling requests, unresolved visit reports.
  10. Schedule new admissions, giving non-clinical information to caregivers.
  11. Monitors missed visits on weekly schedule and ensures documentation is present.
  12. Enters all orders for frequency and duration into computer.
  13. Assigns evaluation visits
  14. May get additional authorization for home care patients
  15. Verifies visits on invoice for Ability Works, passes on to accounts payable
  16. Checks all paperwork (activity logs, etc.) for home care
  17. Reviews and corrects Claims Audit report for accounts receivable
  18. Reviews and corrects Client Schedule report
  19. Receives non-clinical reports from staff. Communicates concern to patient care/case manager as needed.
  20. Coordinates other health related information including but not limited to updating correspondence
  21. Performs hospital follow-up daily, every a.m., update Community Liaisons as needed.
  22. Screens incoming referral calls and forwards to appropriate persons. Obtains basic demographic referral information and forwards to appropriate person.
  23. May assist in maintaining medical supply closet, including inventory and signing out of supplies.
  24. May monitor receipt and mailing of physician’s orders, assessments, plan of care.
  25. May perform technical audit of medical records to ensure proper documents are present.
  26. Tracks visit against documentation received.
  27. Continuously analyzes work processes and makes suggestions for improvement.
  28. Maintains Agency property, supplies and equipment in a manner that demonstrates ownership and accountability.
  29. Assist with receptionist duties as needed. Answers telephone in professional and timely manner. Receives and dispense messages. Handles faxes received and sends, as needed.
  30.   32. Maintains a professional appearance.
  33. Performs other duties as assigned.

Qualifications

Education/Experience: High school diploma or G.E.D. equivalent. Post high school courses in

medical assisting or related area. Minimum of two years work-related experience.

 

Skills/Abilities: Knowledge of medical terminology. Ability to type and perform data entry

accurately. General working knowledge and ability to use office machines and computers.

 Ability to perform several tasks simultaneously, keep accurate records and follows through on

paperwork. Ability to follow instructions and work under time pressure. Demonstrates a high degree

of commitment to customer service and quality work. Exhibits positive interpersonal and telephone

communication skills.
